Delia, I remember I had the same question. 🙂 Just go to my blog on WordPress, and copy the form and change it to add your responses and then nominate 11 other of your followers and then inform them, just like I did. Some people really like to get Awards (I have several and sometimes I post all of them). It really means that you are well thought of and liked very much! Let me know if you have any problems.
